The clinic effectiveness and experience of InterTan intramedullary nail for the treatment of the femoral intertrochanteric fractures in 426 elderly patients

Abstract:Objective To study and evaluate the clinical effectiveness and experience of InterTan intramedullary nail in the treatment of femoral intertrochanteric fractures in elderly patients. Methods Totally 426 elderly patients with femoral intertrochanteric fractures were treated by InterTan intramedullary nail system from Apr.2011 to Oct.2015.Their age ranged from 65 to 93 years (mean,82.5 years).According to Evans-Jensen classification,40 cases were typeⅠ,103 were type Ⅱ,133 were type Ⅲ,89 were type Ⅳ and 61 were type Ⅴ.There were 369 cases with other system diseases.All fractures were reduced and fixed by InterTan intramedullary nail.The operation key points,time,amount of bleeding,postoperative complications and Sanders hip score after 12 months were observed. Results All patients were successfully operated.The time of operation was 45 to 65 minutes and the amount of bleeding was 350 to 450 mL.Three patients gave up on therapy because of cardio-pulmonary function failure,and the other patients recovered well enough.No incision infection was observed after operation.Totally 385 patients were followed up for 12 months after operation and the fractures had been healed.There was no back nail or broken nails besides of 2 cases whose femoral neck joint screw was cut out partly.Sanders hip score was excellent in 116 cases(30.1%),good in 221(57.4%),poor in 48(12.5%) at 12 months after operation,and the excellent and good rate was 87.5%. Conculsion InterTan intramedullar nail is an ideal choice for the treatment of intertrochanteric fracture in the elderly because the holding force of the femoral neck joint locking nail is good and its loosening and cutting out phenomenon is less.
