Comparison of normal and long-type prosthesis for hip replacement in elderly patients with femoral neck fractures

Abstract:Objective To compare the effect of standard and long-type femoral prosthesis in elderly patients with femoral neck fracture.Methods A total of 120 patients of more than or equal to 80 years old with femoral neck fracture were selected in our hospital from Jan.2010 to Jan.2015. There were 66 males and 54 females,with age of 80-93 years old.They were divided into the control group (n=62) and the observation group (n=58) according to different femoral prosthesis.The control group was treated with standard femoral prosthesis,and the observation group was treated with long-type femoral prosthesis.The incidence of postoperative complications and related indicators were compared between the two groups.Results There was no significant difference in the incidence of early postoperative complications between the two groups (P=0.914).The postoperative complications in the observation group were significantly less than those in the control group (P=0.000).On the basis of Delee-Charnley partition method for radiographic assessment of patients in the two groups,the rates of osteolysis and loosening were both 1.79% in the observation group,significantly lower than those of the control group of 14.75% and 16.39%(P=0.012,0.007).Three months and 6 months after surgery,no statistical significance in Harris score between the observation group and the control group was found;but 1 year and 2 years after surgery,Harris hip score in the observation group was (85.21±2.01) and (86.45±2.86),significantly higher than those of the control group [(81.43±1.67) and (82.45±2.09),P=0.000].Conclusion The long-type femoral prosthesis for the treatment of elderly patients with femoral neck fracture has good biological stability,can effectively reduce the incidence of complications and improve the long-term postoperative Harris score.It is safe and reliable,and is worthy of clinical application.
