Effect of delayed open reduction and internal fixation in the treatment of severe Pilon fractures

Abstract:Objective To study the curative effect of delayed open reduction and internal fixation in the treatment of severe Pilon fractures.Methods A total of 144 cases of patients with severe Plion fractures who were admitted from Jun.2010 to May 2014 were as the research object.They were divided into the observation group (72 cases) and the control group (72 cases) according to different treatment methods.The control group was performed early (within 3 days) open reduction and internal fixation,and the observation group was performed delayed (within 7 days) open reduction and internal fixation.Overall efficacy,operation related indexes,Baird ankle joint score,and postoperative complications including traumatic arthritis,bone nonunion,ankle joint stiffness,infection of incisional wound between the two groups were observed and compared.Results The excellent and good rate of the observation group was 91.67%,significantly higher than that of the control group of 79.17% (P<0.05);and the operation time,bleeding volume,postoperative bed-rest time,weight-bearing time,healing time and hospitalization time in the observation group were (62.16±8.23)min,(50.65±6.32)mL,(5.42±1.26)d,(160.73±23.24)d,(115.92±15.33)d,(8.46±1.57)d,respectively,significantly less than those in the control group [(68.94±9.17)min,(74.76±10.17)mL,(8.43±1.19)d,(232.56±30.18)d,(187.49±24.30)d,(13.24±2.21)d],and the differences were all statistically significant(P<0.05).The score of Baird ankle joint of the observation group was (85.13±13.46),(89.50±12.63),(96.15±13.22) after 3 months,6 months and 12 months after surgery,respectively,significantly higher than those of the control group[(74.42±10.89),(78.14±9.87),(80.46±11.78)],and the differences were statistically significant(P<0.05).The total complication rate of the observation group was 4.17%,significantly less than that of the control group of 33.33%,and the difference was statistically significant(P<0.05).Conclusion The delayed open reduction and internal fixation for severe Pilon fractures can contribute to the good recovery of ankle joint function with good efficacy and safety.This method is worth clinical application.
