Comparison of two kinds of methods in the treatment of closed femoral subtrochanteric fracture (Russell-Taylor type Ⅱ)

Abstract:Objective To evaluate the clinical effect of the closed femoral subtrochanteric fracture (Russell-Taylor type Ⅱ) with locking proximal femoral plate (LPFP) or proximal femoral nail antirotation (PFNA).Methods From Jan.2011 to Aug.2013,110 cases with closed femoral subtrochanteric fracture (Russell-Taylor Ⅱ type) were treated with the following two methods: LPFP (65 patients) and PFNA (65 patients).The peri-operative index levels,the improvement of hip joint function,the hip joint activity,the post-operative complication were compared between the two methods.Results The incision length,operative time,intraoperative exposure times,intraoperative blood loss in the PFNA group[(6.55±1.34)cm,(68.41±7.50)min,(11.53±2.93)mSv and (163.30±24.79)mL] were better than those in the LPFP group [(10.93±2.24)cm,(77.80±9.35)min,(19.10±4.63)mSv,(220.14±35.62)mL],and the differences were statistically significant (P<0.05).Healing time of the two groups were (3.76±1.05) months and(3.38±0.90)months,respectively,and the difference was not statistically significant.The differences of the postoperative hip joint function,activity and the postoperative complication were not statistically significant(P>0.05).Conclusion LPFP and PFNA in treatment of femoral subtrochanteric fracture can achieve similar effect to promote recovery process of hip joint function.However,PFNA has advantages including simple operation,minimally invasiveness and rapid postoperative recovery.
