Treatment experience of occult traumatic aortic rupture

Abstract:Objective To investigate the diagnosis and treatment of occult traumatic aortic rupture and to improve the understanding and capability of dealing with it .Methods From Mar.2010 to Mar.2015,15 cases with occult traumatic aortic rupture were in the 175th hospital of PLA.Their clinical features, imageological examinations and treatment were analyzed retrospectively .There were 12 males and 3 females,aged between 18 and 52 years old (mean 37.6 years).All were injured from traffic accidents.All 15 cases were associated with pulmonary contusion and hemothorax and 5 were associated with limb fracture .Results Three cases died in 1 hour after admission to hospital(no operation was performed).Endovascular graft exclusion (EVGE),thoracotomy and medical treatment succeeded in treating 7 cases,2 cases and 1 case respectively.All the patients were followed up for 6 months to 5 years and all of them were still alive .Conclusion Strengthening the understanding of occult traumatic aortic rup-ture,explicit diagnosis ,immediate and efficient treatment ,and the use of endovascular graft can improve the success rate of rescue and reduce the mortality and complications .
