Partial corpectomy with titanium mesh reconstruction for old lower cervical fracture and dislocation

Abstract:Objective To evaluate the efficacy of partial anterior cervical corpectomy ( PACC) reconstructed with autograph packed titanium mesh for the treatment of old lower cervical fracture and dislocation .Methods A retrospective study was conducted for 30 patients treated with PACC and titanium mesh between Jan .2006 and Jan. 2014 for old lower cervical fracture and dislocation associated with spinal cord injury .Among them 25 were males and 12 were females with an average age of 42(19-61) years.The JOA Score of spinal cord function was assessed by JOA score.The average JOA score of the eight cases with complete injury was 1 point and of the 29 cases with incomplete injury was 5 points.Results The average follow-up was 14(7-33) months.All fused segments achieved bony union within 5 months on radiographic evaluation .No breakage or loosening of the titanium mesh or screw happened .The JOA score of spinal cord function improved from 5 to 13 for those patients with incomplete spinal cord injury .Patients with complete spinal cord injury had nerve root function improvement at injury level .Conclusion PACC reconstruc-ted with autograph packed titanium mesh for the treatment of old lower cervical fracture and dislocation can bring a -bout full decompression ,sequence and physiological curvature of the cervical vertebra ,immediate and long term ante-rior cervical column support ,and nerve function restoration .
