A comparative study on effect of bacterial clearance in burn wound with compound lysostaphin

Abstract:Objective To observe the effects of compound lysostaphin on burn wound infections caused by common antibiotic resistant bacteria colonization.Methods Totally 120 patients in our burn ward were enrolled and were randomly divided into two groups,with 60 patients in each group.Several layers of gauze,either soaxed with compound lysostaphin in group A or 0.5% povidone-iodine in group B,were used to cover the burn wounds once a day,with continuous use until wound healing.Wound swab cultures were carried out before and 24h,72h af-ter the change of dressing.The bacterial species and their total number of colonies were identified.Adverse reac-tions were observed before and after treatment,and also the hepatic and renal functions of the patients were examined before and after the change of dressing.The examinations of hepatic and renal function were conducted and the heal-ing time of the burn wounds and adverse reactions were recorded.Results The dominating bacteria in the burn wounds in both groups were MRSA,Pseudomonas aeruginosa,Acinetobacter baumannii and Escherichia coli.All four kinds of bacterial load in two groups were high before dressing (>105cfu/cm2).24h,72h after dressing, the total number of colonies in group A dropped to 40 cfu/cm2 or less,while the number of MRSA,Pseudomonas aeruginosa and Acinetobacter baumannii in group B was still 103 cfu/cm2 or more.The total number of colonies was significantly different between group A and group B(P<0.05).The wound healing time in group A (18.85 ±2.53 days) was significantly shorter than that in group B [(24.01 ±2.16)d,P<0.05].The hepatic and renal functions in both groups were normal without any adverse reaction.Conclusion Compound lysostaphin has a wide bactericidal spec-trum capacity and strong bactericidal capability,and can effectively clear microbe and promote wound healing.It is worth promoting the use of microbial colonization and infection control in burn chronic wound.
