Clinical analysis of buttress plate fixation for posterior Pilon fracture through a combined posterolateral and posteromedial approach

Abstract:Objective To investigate the clinical efficacy of buttress plate internal fixation via a combined posteromedial and posterolateral approach for Pilon fracture .Methods Twenty patients with Pilon fracture were treated with buttress plate internal fixation via a combined posteromedial and posterolateral approach .Fifteen pa-tients were followed up outside the hospital ,including 9 males and 6 females,with age ranging from 27-70 years(av-erage,45.7 years).Results At postoperative 3 days,X-ray film showed anatomic reduction (no distal tibial artic-ular surface,normal ankle mortise) in 11 cases,good reduction (distal tibial articular surface displacement <1mm, widened ankle hole <1mm) in 3 cases,and the excellent and good rate was 93.3%.Among 15 cases receiving reg-ular follow-up visits,the follow-up time was 3-36 months (mean 16 months).All the fractures were healed.The av-erage healing time was 15 weeks ( 12-17 weeks ) , and the full weight bearing time was 17-24 weeks ( mean 22 weeks).According to Mazur ankle function scoring system ,the ankle function score at postoperative 1 year was ex-cellent in 11 cases,good in 3 cases,good in 1 case,and the mean score was (95 ±3)points.One case developed su-perficial infection of incision and was cured after non-surgical treatment.No case had screw loosening ,fracture,in-ternal fixation failure or other complications .Conclusion Buttress plate internal fixation via a posteromedial and posterolateral combined approach can provide safe and easy reduction and fixation for Pilon fracture .
