Intra-abdominal volume increment combined with VSD in the treatment of intra-abdominal hypotension/abdominal compartment syndrome

Abstract：Objective To investigate the effect of continued VSD applied in intra-abdominal hypertension (IAH) /abdominal compartment syndrome (ACS) patients undergoing open abdomen (OA) and intra-abdominal volume increment(IAVI).Methods The clinical data of 38 cases with IAH/ACS treated from Jun.2009 to Aug. 2015 were retrospectively analyzed .All patients had IAH or ACS basis (IAP was>18mmHg,or combined with fail-ure of one or more organs).A negative pressure of 60-80mmHg was maintained.Continuous negative drainage in 24h was obtained and routine monitoring of bladder pressure was conducted .After 4-14d,the intra-abdominal pres-sure was <12mmHg during continuous 48h and the abdominal cavity was closed with incisional suture .If the intra-abdominal pressure continued >12mmHg and the incision cannot be closed ,or though the intra -abdominal pres-sure was <12mmHg,the general condition cannot sustain an operation ,VSD was replaced according to the patient ’s condition,in which skin grafting might be applied to close the abdominal wound ,forming a planned abdominal herni-a.Results Of the 38 patients,31 cases were successfully treated and 7 died.Twenty-five cases achieved defini-tive abdominal closure within 4-14 days and 6 cases who received skin grafting and continuous application of VSD had their wounds healed ,including 5 cases of planned abdominal factors .Conclusion IAVI combined with VSD is an effective surgical method in the treatment of IAH/ACS caused by multiple factors .
