Clinical effect of dynamic hip screw and proximal femoral nail anti-rotation in the treatment of femoral intertrochanteric fracture

Abstract:Objective To compare the clinical effect of dynamic hip screw ( DHS) and proximal femoral nail an-ti-rotation(PFNA) in the treatment of femoral intertrochanteric fracture .Methods From Jan.2011 to Jan.2014, fifty-eight patients with femoral intertrochanteric fracture were admitted into our department . Excluding the 4 pa-tients with conservative treatment,54 were treated with DHS(DHS group,n=20)and PFNA(PFNA group,n=34). The incision length,length of hospital stay,operation time,intraoperative bleeding volume,volume of drainage,frac-ture healing time and postoperative hip function and complications were compared between the two groups .Results The difference of the operation time , intraoperative bleeding volume ,length of stay ,postoperative data such as the volume of drainage and the average fracture healing time between the two groups had statistical significance ( P<0.05).The PFNA group was better than the DHS group .There was no statistically significant difference between the two groups in the incision length(P>0.05).The Harris score in the DHS group was 85%while 91.2%in the PFNA group.Conclusion PFNA in the treatment of femoral intertrochanteric fracture has the advantages of easy operation,short operation time,little blood loss,small trauma and fast healing.It is suitable for almost all types of intertrochanteric fractures , especially for eldely osteoporosis fracture patients ,unstable fracture patients and paitents with complications .
