Clinical experience of two kinds of kirschner wire tension belt fixation in the treatment of Wadsworth type ⅡandⅢolecranon fracture

Abstract:Objective To investigate the clinical efficacy and safety differences of two kinds of kirschner wire tension belt fixation in the treatment of Wadsworth typeⅡ and Ⅲ olecranon fracture via medullary cavity and double cortex.Methods Forty patients with Wadsworth typeⅡandⅢolecranon fracture were chosen in the peri-od from Jan.2012 to Dec.2013 in our hospital and randomly divided into group A and group B,with 20 patients in each group.In group A,there were 13 males and 7 females,with an average age of (34.64 ±5.72) years (20-48 years);in group B,there were 14 males and 6 females,with an average age of (34.58 ±5.70)years (19-48 years). Patients in group A were treated with kirschner wire tension belt fixation via medullary cavity,and patients in group B were treated with kirschner wire tension belt fixation via double cortex.The elbow flexion and extension activity, forearm rotation activity,excellent and good rate of elbow joint function score and postoperative complication rate of both groups were compared.Results There was no significant difference in the excellent and good rate of elbow joint function score,elbow flexion and extension activity and forearm rotation activity between the two groups(P>0.05).The postoperative complication rate of group B was significantly better than that of group A(P<0.05).Conclusion Kirschner wire tension belt fixation in the treatment of Wadsworth type Ⅱ and Ⅲ olecranon fracture can efficiently promote the recovery of limb activity and improve the function of elbow joint.Compared with kir-schner wire tension belt fixation via medullary cavity, kirschner wire tension belt fixation via double cortex in the treatment of Wadsworth typeⅡandⅢolecranon fracture possesses lower postoperative complication risk.
