Medial posterior approach for treating humeral shaft fracture

Abstract:Objective To investigate the clinical effect of open reduction and plate internal fixation via me-dial posterior approach in treating humeral shaft fracture.Methods Twenty patients were treated with open reduc-tion and plate internal fixation via medial posterior approach from Oct.2012 to Apr.2014,including 14 males and 6 females.Their age ranged from 16 to 45 years,with an average of 31 years.Four patients were injured on the left humeral side and 16 patients on the right humeral side.According to the fracture line classification,there were 2 ca-ses of horizontal fracture,8 cases of spiral fracture,3 cases of oblique fracture,and 7 cases of comminuted fracture. Patients took elbow joint functional exercise under guidance of medical staff 3 days after operation.Results All the twenty patients were followed up for 10-26 months,averaging 12 months.The fracture healing time ranged from 6 to 8 months,steel plates were removed 8-11 months after operation,without nerve and blood vessel injury.All the 20 patients achieved excellent elbow and shoulder function recovery.Conclusion The open reduction and plate inter-nal fixation via medial posterior approach for treating humeral shaft fracture has advantages of safe nerve exposure, easy fracture reduction,strong fixation,rapid fracture healing,hidden incision and early upper limb functional exer-cise.It is worth of wide application because of its good clinical effect.
