Application of three-dimensional CT reconstruction techniques in individualized total knee replacement

Abstract:Objective To discuss the clinical effects of using three-dimensional CT reconstruction tech-niques in developing individualized knee model and to explore the combining site to provide basis for the individual -ized knee replacement surgery .Methods CT-scan of lower limbs of 72 cases of severe knee osteoarthritis from Mar.2011 to May 2014 were done to complete the three-dimensional model of the lower limbs .Critical angle and bone cutting data were achieved to set individualized bone cutting plan .The best power line of the lower limbs re-construction,osteotomy parameters and placement angle of prosthesis were achieved .Data was applied during the operations .The degree of hip-knee-ankle angle and the standard deviation and HSS on the second week before and after the operation were recorded and the excellent rate was calculated .Results The lower limb power lines of the patients were close to ideal power lines .The degree of hip-knee-ankle angle and the standard deviation was 8.1482° ±1.5957°before surgery, while 1.0627°±0.6401°after surgery. Paired t-test was used ( t =13.6683, P =0.039),which had statistical significance .HSS score of the knee joints was improved from (49.6 ±6.1) points be-fore surgery to (88.2 ±4.1) points two weeks after surgery with statistical significance (P=0.041).According to the HSS score after surgery,there were 58 excellent cases,10 good,3 fair and 1 poor.The excellent and good rate was 94.44%.Conclusion Three-dimensional CT reconstruction techniques combined with measurement of impor -tant parameters and plan making before surgery can reduce mistakes and improve surgery accuracy .It is helpful to reconstruct lower limb power line correctly ,cut bone accurately ,and implant prosthesis .It can also provide clinical basis for realizing the individualization and standardization of total knee arthroplasty .
