Causes and therapeutic strategy of postoperative breakage of pedicle screws in the treatment of thoracolumbar burst fractures

Abstract:Objective To analyze the causes and strategy of postoperative breakage of pedicle screws in the treatment of thoracolumbar burst fractures and its countermeasures.Methods Retrospective analysis of pedicle screw breakage found in 17 out of 286 cases of thoracolumbar burst fractures from May 2005 to Aug.2011 was carried out. There were 14 males and 3 females.Fourteen cases ranged from 24 years to 44 years and 4 ranged from 45 years to 60 years.Among those underwent fixation with pedicle screws 7 suffered nail breakage.Ten patients did not receive fixa-tion.Nail breakage occurred in 1 case in the intervertebral bone graft group,7 cases in the posterior bone graft group and 9 cases in none bone graft group.Fourteen cases underwent decompressive laminectomy and 3 cases did not.Re-sults In each independent factor of the breakage of pedicle screw,gender did not have statistical significance(P>0.05).High risk factors of pedicle screw breakage included age of more than 45 years old,no rail fixation,no fixation in injured vertebrae,laminectomy,decompressive laminectomy,no bone graft and burst loose area of more than 50%. They all had statistical significance(P<0.05).Conclusion For patients with risk factors of thoracolumbar burst fractures,individual measures and targeted treatment should be performed to prevent pedicle screw breakage.
