Application value of damage control surgery in patients with gradeⅣandⅤliver trauma in primary hospitals

Abstract:Objective To conduct a retrospective comparison of postoperative mortality between damage control surgery and definitive surgery in the treatment of grade Ⅳ and Ⅴ severe liver trauma in primary hospitals and to evaluate the effect of the two treatments .Methods Thirty-eight patients with severe hepatic trauma in our hospital from Jan.2007 to Nov.2013 were analyzed retrospectively .According to AAST grade,there were 23 cases of grade Ⅳ,15 cases of gradeⅤ.Thirty-five cases were associated with other kinds of organic injuries .Twenty-two cases were operated with damage control surgery and 16 cases were given definitive operation .Comparison and anal-ysis of the two groups in mortality were conducted .Results Differences of the two groups in patient age ,injury time,injury classification ,shock index and the amount of preoperative blood loss had no statistically significance ( P>0.05).Compared with the definitive operation group ,however,the mortality and operation time in the damage control surgery group reduced significantly and the difference was statistically significant (P<0.05).Conclusion Damage control surgery in the treatment of grade ⅣandⅤsevere liver trauma in primary hospitals can be effective in reducing patient risk of death with good clinical application value .
