Clinical efficacy of posterior monosegmental pedicle screw fixation in the treatment of thoracolumbar fracture

Abstract：Objective To analyze the security and clinical effects of pedicle bone graft combined with pos-terior monosegmental pedicle screw fixation for traumatic thoracolumbar fracture . Methods Eighty patients with traumatic thoracolumbar fracture from Jan .2011 to Jun.2013 in our hospital were selected and divided into experi-mental group and control group .The experimental group received vertebrae pedicle bone graft combined with poste -rior monosegmental internal fixation with pedicle screw .The control group only received posterior monosegmental in-ternal fixation with pedicle screw .Reposition,IVW,sagittal index and VAS scores of the two groups before and after treatment were compared . Oswestry and complications classification after six months of treatment were also ob-served.Results The horizontal displacement ,sagittal and frontal plane Cobb angle of the two groups at 6 months after the surgery were significantly less than those before the treatment ,while the injured vertebral height was signifi-cantly higher than that before the treatment (P<0.01).The horizontal displacement,sagittal and frontal plane Cobb angle of the experimental group 6 months after the surgery were significantly lower than those of the control group , while the injured vertebral height was significantly higher than that of the control group (P<0.01).The IVW,sagit-tal index and VAS scores of the two groups were significantly lower than those before the treatment (P<0.01).The IVW,sagittal index and VAS scores of the experimental group at 6 months after the surgery were significantly lower than those of the control group(P<0.01).Oswestry grade of the experimental group after six months was signifi-cantly better than that of the control group ( P<0.05 ) . The incidence of postoperative complications in the two groups were not significantly different (P>0.05).Conclusion Vertebrae pedicle bone graft combined with poste-rior monosegmental internal fixation with pedicle screw has good clinical efficacy for traumatic thoracolumbar frac -ture.It can significantly improve the prognosis of patients without increasing the risk of surgery ,and is worthy of clinical application .
