Evaluation of application of composite type external fixator in treatment of high-energy tibial plateau fracture

Abstract:Objective To evaluate the results of composite type external fixator in treatment of high-energy tibial plateau fracture.Methods From Jan.2008 to Jan.2012,17 cases of high-energy tibial plateau fracture were treated with composite type external fixator combined with limited internal fixation .There were 6 cases of type Ⅴand 11 cases of type Ⅵaccording to Schatzker's classification.By Gustilo classification, there were 6 cases of typeⅡ,7 cases of type ⅢA and 4 cases of type ⅢB.Results All patients were followed up for an average of 17.6 months(range 9 to 30).All the fractures healed after an average time of 7.2 months.Evaluation by the Holh knee functional scoring system showed excellent in 10 cases,good in 3,fair in 3 and poor in 1.Three patients had their external fixators replaced with plates and iliac bone graft to get fracture union .The wounds of 6 cases healed prima-rily,and 11 cases got a secondary healing by VSD vacuum aspiration and free flap transplantation or pedicled flap transposition without any serious complications ,such as infection.Conclusion The composite type external fixator for treating high-energy tibial plateau fractures can achieve early functional exercise and is an ideal method ,especial-ly for patients combined with severe soft tissue injury .
