Diagnostic value of combined color doppler ultrasound,serum pro-GRP,and CD134 detection for tuberculous pleurisy

Abstract:Objective To evaluate the diagnostic value of color Doppler ultrasound(CDU)combined with serum pro-gastrin releasing peptide(pro-GRP)and CD134 for tuberculous pleuritis(TP).Methods A total of 196 patients with pleural effusion who were admitted to our hospital from May 2020 to May 2024,96 patients with TP diagnosed by biopsy puncture or culture of pleural effusion with Mtb were selected as the observation subjects(TP group),and 100 patients with non-TP pleurisy were selected as the control group.Enzyme linked immunosorbent assay was used to measure the expression levels of serum pro-GRP and CD134.ROC curve was used to analyze the diagnostic efficacy of serum pro-GRP and CD134 levels for TP.Kappa test was used to analyze the consistency between CDU combined with serum pro-GRP and CD134 in diagnosing TP and clinical pathological results.Results Compared with the control group,peak systolic blood flow velocity(PSV)index,serum pro-GRP,CD134 levels and the percentage of T-SPOT.TB positivity increased and resistance index(RI)decreased in the TP group(P<0.05).ROC curve results showed that the AUC,sensitivity,and specificity of serum pro-GRP for diagnosing TP were 0.842%,76.04%,and 86.00%,respectively.The AUC,sensitivity,and specificity of serum CD134 for diagnosing TP were 0.849%,81.25%,and 76.00%,respectively.The consistency between the CDU,serum pro-GRP,and CD134 individual diagnosis and clinical results showed Kappa values of 0.705,0.622,and 0.572,respectively.There were 5 false positives and 10 false negatives in the combined diagnosis of TP by CDU,serum pro-GRP,and CD134,with a high consistency with pathological results and a Kappa value of 0.847(P<0.05).The specificity,accuracy,and positive predictive value of the combination of CDU,serum pro-GRP,and CD134 in diagnosing TP were higher than those of CDU,pro-GRP,and CD134 alone(P<0.05).Conclusions Serum pro-GRP and CD134 levels are abnormally upregulated in TP patients.The combination of CDU and serum pro-GRP and CD134 detection can improve the specificity and accuracy in early diagnosis of TP.
