Study on the relationship between the levels of pulmonary surfactant protein D,high mobility group protein B1,and red blood cell distribution width and the severity and prognosis of macrolide resistant Mycoplasma pneumoniae pneumonia in children

Abstract:Objective The study aimed to analyze the relationship between serum surfactant protein D(SP-D),high mobility group protein B1(HMGB1)and peripheral red blood cell distribution width(RDW),and the severity and prognosis of children with macrolide-resistant Mycoplasma pneumoniae pneumonia.Methods A total of 106 children with macrolide-resistant Mycoplasma pneumoniae hospitalized between June 2021 and June 2024 were selected as the observation group,including 42 cases with mild symptoms and 64 cases with severe symptoms.A total of 106 children with macrolide non-resistant Mycoplasma pneumoniae pneumonia were selected as the control group.The serum SP-D,HMGB1 and peripheral blood RDW levels of all the selected subjects were measured,the disease progression and poor prognosis of the observation group were recorded,and the receiver operating characteristic curve(ROC)was used to analyze the predictive performance of serum SP-D,HMGB1 and peripheral blood RDW on poor prognosis.Results Compared with the control group,the serum SP-D,HMGB1 and peripheral blood RDW levels in the observation group were higher(P<0.05),the serum SP-D,HMGB1 and peripheral blood RDW levels in the observation group were higher than those in the mild group(P<0.05),the serum SP-D,HMGB1 and peripheral blood RDW levels in the disease progression group were higher than those in the stable group(P<0.05).The sensitivity and specificity of serum SP-D and HMGB1 combined with peripheral blood RDW in predicting the poor prognosis of children with macrolide-resistant Mycoplasma pneumoniae pneumonia were 91.82%,62.76%,and the area under the ROC curve(AUC)was 0.916.Conclusions SP-D,HMGB1 and RDW are closely related to the severity of disease in children with macrolide-resistant Mycoplasma pneumoniae pneumoniae,and the combined application of the three can help improve the predictive performance of poor prognosis in children,which is worthy of further research and application.
