Analysis of pathological characteristics and drug resistance of different Helicobacter pylori genotypes in gastric cancer patients

Abstract:Objective To explore the infection rate and classification of Helicobacter pylori(Hp)in gastric cancer patients,and to explore the drug resistance of Hp in gastric cancer patients.Methods A total of 103 gastric cancer patients treated in our hospital from June 2019 to June 2024 were selected as study subjects.Venous whole blood samples were collected to detect Hp antibodies,and the gastric cancer patients were divided into Type Ⅰ group,Type Ⅱ group,and negative group.The pathological characteristics among the three groups were compared.Gastric antral tissue samples were collected via gastroscopy for drug sensitivity testing.Results The Hp infection rate among the 103 gastric cancer patients was 75.73%,with Type Ⅰ Hp accounting for 66.02%and Type Ⅱ for 9.71%.Among Hp-positive gastric cancer patients,those with CagA+VacA antibodies were the most prevalent,accounting for 40.78%,followed by those with Vac A antibodies at 19.42%.Disease stage,depth of invasion and lymph node metastasis in type Ⅰ and Ⅱ were significantly higher than the negative group(P<0.05),while there was no significant difference between type Ⅰ and type Ⅱ(P>0.05).Hp in gastric cancer patients exhibited certain drug resistance to metronidazole and clarithromycin,with resistance rates of 51.28%and 32.05%,respectively.They were more sensitive to levofloxacin,amoxicillin,and moxifloxacin,with resistance rates of only 3.85%,5.13%,and 7.69%,respectively.There was no significant difference in Hp drug resistance between the Type Ⅰ and Type Ⅱ groups(P>0.05).Conclusion Gastric cancer patients have a high Hp infection rate,and there are significant differences in pathological characteristics among gastric cancer patients with different Hp genotypes.Clinical attention should be given to Hp detection in gastric cancer patients,and sensitive drugs such as levofloxacin and amoxicillin should be used targetedly.
