Analysis of influencing factors of latent tuberculosis infection in patients with type 2 diabetes mellitus

Abstract:Objective To investigate the influencing factors of Latent tuberculosis infection(LTBI)in patients with Type 2 diabetes mellitus(T2DM),and to provide theoretical basis for early clinical identification and screening of tuberculosis infection.Methods A total of 175 T2DM patients who completed tuberculosis interferon gamma release test and had positive results from January 2021 to January 2024 at the First Affiliated Hospital of Xinjiang Medical University were selected as the LTBI group,and 465 T2DM patients who had negative interferon gamma release test results during the same period were selected as the control group.The clinical data of patients were collected,including gender,age,ethnicity,education,smoking history,drinking history,disease duration,whether hypoglycemic treatment and laboratory test indicators.Independent sample t-test or rank sum test is used for inter group comparison of quantitative data,chi square test is used for count data,and Logistic regression analysis is used for multivariate analysis.Results There were significant differences in gender and nationality between LTBI group and control group(P<0.05).The levels of monocyte count,C-reactive protein,glycosylated hemoglobin and complement C3 in the LTBI group were higher than those in the control group,and the differences were statistically significant(P<0.05).Multivariate analysis showed that male(OR=2.866,95%CI:1.797-4.571),Uygur ethnicity(OR=9.971,95%CI:6.050-16.433),monocyte count(OR=2.706,95%CI:1.029-7.114),complement C3(OR=5.862,95%CI:1.905-18.045)and glycosylated hemoglobin(OR=1.106,95%CI:1.004-1.219)were independent risk factors.Conclusion LTBI screening should be strengthened in male and Uygur patients with T2DM.The increase of monocyte count,glycosylated hemoglobin and complement C3 may increase the risk of LTBI infection in T2DM patients.
