Coagulation function indicators combined with AMIS65 scoring system predict the risk of upper gastrointestinal bleeding in patients with liver cirrhosis

Abstract:Objective To analyze the predictive value of coagulation function indicators[PT,fibrinogen(FIB),thrombin time(TT),activated partial thromboplastin time(APTT)]combined with AMIS65 scoring system for the risk of upper gastrointestinal bleeding in patients with cirrhosis.Methods Two hundred and the patients with liver cirrhosis diagnosed and treated(upper gastrointestinal bleeding group in 60 cases and non-upper gastrointestinal bleeding group in 150 cases)were enrolled in the First Affiliated Hospital of Hebei North University between January 2021 and January 2023.Logistic regression analysis was carried out on the factors affecting the patients with cirrhosis complicated with upper gastrointestinal bleeding;the predictive value of ROC curve analysis of coagulation function indicators combined with the AMIS65 scoring system for upper gastrointestinal bleeding in patients with liver cirrhosis.Results Compared with the non-upper gastrointestinal bleeding group,the levels of PT,TT and APTT in the upper gastrointestinal bleeding group were obviously increased,while the level of FIB was obviously decreased(P<0.05);the AMIS65 score was significantly increased(P=0.000).Logistic regression analysis showed that liver function liver classification,esophageal and gastric varices,ascites,history of hemorrhagic disease,PT,TT,APTT,AMIS65 scores were the risk factors of upper gastrointestinal bleeding in patients with cirrhosis(P<0.05),while FIB was the protective factor for patients with upper gastrointestinal bleeding(P=0.000);the AUC of the combined detection of FIB,TT,APTT,and AMIS65 score in predicting cirrhosis complicated with upper gastrointestinal bleeding was 0.957,which was better than their individual prediction(Zcombined detection-FIB=6.200,P=0.000;Zcombined detection-TT=5.431,P=0.000;Zcombined detection-APTT=4.516,P=0.000;Zcombined detection-AMIS65 score=3.634,P=0.000).Conclusion The TT,APTT and AMIS65 score in patients with cirrhosis complicated with upper gastrointestinal bleeding increase,and the level of FIB decreases,which have certain value in the risk assessment of cirrhosis complicated with upper gastrointestinal bleeding.
