Characterization of occult hepatitis B virus infection in Chongqing unpaid blood donors

Abstract:Objective To investigate the prevalence of occult HBV infection (OBI) of blood donors in Chongqing, and to analyze the characteristics of its serology and virology.Methods A total of 93625 prescreening acceptable blood samples were screened for HbsAg, anti-HCV and anti-HIV by EIAs, and tested for HBV, HCV and HIV-1 nucleic acids by nucleic acid test (NAT). HBsAg(-)/HBV DNA(+) samples were detected for anti-HBc, anti-HBs, anti-HBe and HBeAg. And the anti-HBc(+) samples were further tested for viral loads and genotyping. Results The detection rate of HBsAg(-)/HBV DNA(+) was 0.097% (91/93625). Among 91 samples, 79 were positive for anti-HBc, the detection rate was 0.084% (79/93625). These donors were called OBI. The OBI samples could be divided into 4 types according to serological tests, which were anti-HBc(+), anti-HBc(+)/anti-HBs(+), anti-HBc(+)/anti-HBe(+) and anti-HBc(+)/anti-HBs(+)/anti-HBe(+). According to the result of real-time fluorescent quantitative PCR(QPCR), all specimens of OBI′s viral load was 0-1056.8 IU/ml (median 108.6 IU/ml). The genotype was mainly B.Conclusions OBI′s detection rate is higher in unpaid blood donors in Chongqing area, and the serology and virology characteristics are regionally. NAT can improve the sensitivity of OBI detection, but some results are false-positive, which have important effects on the blood safety.
