Effect of endoscopic therapy for recurrent gastroesophageal variceal hemorrhage on hemodynamics of portal venous system

Abstract:ObjectiveTo evaluate the effect of endoscopic therapy for recurrent gastroesophageal variceal hemorrhage (secondary prophylaxis) on hemodynamics of portal venous system so as to provide reference for choosing treatment modalities.MethodsColor Doppler ultrasound was used to evaluate portal pressure and hemodynamic indexes of the portal venous system including internal diameter, cross area and mean blood flow velocity in patients with gastroesophageal variceal hemorrhage, and evaluate the hemodynamic indexes of the portal venous system after eradication of gastroesophageal varices with endoscopic therapy for a second time.ResultsA total of 42 patients with cirrhosis and portal hypertension who underwent endoscopic therapy for recurrent gastroesophageal variceal hemorrhage achieved eradication of gastroesophageal varices during 1-27 months of follow-up. The evaluation on hemodynamics of the portal venous system using color Doppler ultrasound showed that the inner diameter and cross area of hepatic artery widened significantly after endoscopic therapy, and the differences in other evaluation indexes were not significant before and after endoscopic therapy. ConclusionsEndoscopic therapy for gastroesophageal variceal hemorrhage is likely to increase the portal pressure, which should be verified by large-sample trials and long-term follow-up.
