Application of Continuity Care Model Based on Smart Medical Platform in Patients with Chronic Obstructive Pulmonary Disease
Abstract:Objective: To explore the application effect of continuity care model based on smart medical platform in patients with chronic obstructive pulmonary disease (COPD). Methods: A total of 95 COPD patients admitted to the respiratory department of Yancheng People's Hospital from January to June 2023 were selected as the study subjects. The 48 patients admitted from January to March 2023 were set as the control group, and the 47 patients admitted from April to June 2023 were set as the observation group. The control group received routine continuity care, while the observation group received the continuity care model based on the smart medical platform. The compliance with pulmonary rehabilitation, pulmonary function indicators, and quality of life were compared between the two groups. Results: After 3 months of intervention, the compliance with pulmonary rehabilitation in the observation group was higher than that in the control group (t=3.108, P=0.003). The forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), and the ratio of FEV1 to FVC were all better in the observation group than in the control group (t=1.041, P=0.032; t=11.020, P<0.001; t=2.131, P<0.001). The scores of all dimensions of quality of life were higher in the observation group than in the control group (all P<0.05). Conclusion: The continuity care model based on the smart medical platform can improve the compliance with pulmonary rehabilitation in COPD patients, improve the pulmonary function indicators, and enhance the quality of life of patients.
