Analysis of the Trajectory of Psychological Resilience in Patients After Transurethral Resection of Bladder Tumor (TURBT)
Abstract:Objective To analyze the trajectory of psychological resilience in patients after transurethral resection of bladder tumor (TURBT) using latent class growth modeling (LCGM). Methods A total of 214 bladder cancer patients treated at the First Affiliated Hospital of Soochow University from January 2020 to August 2024 were selected. All patients received TURBT and the Connor-Davidson Resilience Scale (CD-RISC) assessment. LCGM was used to analyze the trajectory of psychological resilience, and multiple linear regression was used to identify factors influencing psychological resilience. Results The total scores of CD-RISC before surgery and at 1, 3, and 6 months post-surgery were (54.76±4.78), (60.78±6.69), (55.74±5.47), and (53.32±4.31) points, respectively. LCGM identified three classification trajectories as the optimal model, divided into the vulnerable group, stable group, and growth group. There were statistically significant differences in the total CD-RISC scores among the three groups before and after surgery at 1, 3, and 6 months (P < 0.05). At 6 months post-surgery, there were statistically significant differences in CD-RISC scores among patients with different levels of education, metastasis, social support, and family function (P < 0.05). Education level, social support, and family function were protective factors for psychological resilience after bladder cancer surgery, while metastasis was a risk factor (P < 0.05). Conclusion The evolution of psychological resilience after TURBT for bladder cancer shows group differences. Clinically, dynamic assessment should be used to promptly identify individual trajectory types (vulnerable/stable/growth) and implement tailored psychological intervention plans.
