Lixinqi Continuing Medical Education Garden - Thinking Case Series (7) Answers
Abstract:The case is presented on the color page of this issue. Diagnostic thinking: The child was initially diagnosed with viral encephalitis at another hospital, and came to the hospital for rehabilitation due to limited upper limb activity. However, during the physical examination, it was found that the child had a wrist drop in the distal part of the upper limb, which does not match the signs of upper motor neuron paralysis sequelae from viral encephalitis. A detailed medical history revealed that the child had a history of delayed motor development compared to peers. After an acute infection, the child experienced epileptic seizures and limited movement of the right limb. Auxiliary examinations showed protein-cell dissociation in the cerebrospinal fluid, abnormal signals in the left thalamus on MRI, and electromyography indicating impaired peripheral nerve motor conduction function. It is considered that both central and peripheral nerves are involved.
