Successful Rescue of Two Adult Patients with Severe Psittacosis Chlamydial Pneumonia Using Extracorporeal Membrane Oxygenation: A Case Report and Literature Review
Abstract:This article reviews and analyzes in depth the diagnostic and therapeutic processes of two adult patients with severe psittacosis chlamydial pneumonia, exploring the clinical application value of extracorporeal membrane oxygenation (ECMO) combined with metagenomic next-generation sequencing (mNGS) in the treatment of this disease. Both patients presented with fever, cough, and dyspnea as initial symptoms, and their conditions rapidly deteriorated, eventually progressing to severe pneumonia. When conventional treatment failed, both patients were initiated on veno-venous ECMO support within 48 hours of admission, and mNGS technology quickly identified the pathogen as Chlamydia psittaci. Subsequently, the treatment regimen was adjusted to include doxycycline-based antimicrobial therapy, and both patients successfully weaned off ECMO and were discharged after recovery. Combined with a literature review, early ECMO intervention combined with rapid mNGS diagnosis can help improve the survival rate of patients with severe psittacosis. Therefore, it is recommended that in the management of severe pneumonia, attention should be paid to the history of contact with poultry, and the timing for ECMO intervention should be assessed as early as possible for patients with rapidly progressing conditions, while combining mNGS for precise treatment.
