Clinical Observation of Modified Huangqi Guizhi Wuwu Decoction Combined with Empagliflozin in the Treatment of Type 2 Diabetes Mellitus
Abstract:Objective: To observe the efficacy of modified Huangqi Guizhi Wuwu Decoction combined with empagliflozin in the treatment of type 2 diabetes mellitus (T2DM). Methods: A total of 102 T2DM patients admitted to Xiangcheng Hospital of Traditional Chinese Medicine from January 2023 to January 2025 were selected as the study subjects and randomly divided into a monotherapy group (treated with empagliflozin) and a combination group (treated with modified Huangqi Guizhi Wuwu Decoction combined with empagliflozin), with 51 cases in each group. The levels of oxidative stress indicators, vascular function regulatory factors, peripheral nerve conduction velocity, TCM syndrome scores, clinical efficacy, and incidence of adverse reactions were compared between the two groups before and after treatment. Results: After treatment, the serum levels of malondialdehyde (MDA) and oxidized low-density lipoprotein (Ox-LDL) in the combination group were lower than those in the monotherapy group, while the glutathione peroxidase (GSH-Px) level was higher, with statistically significant differences (P < 0.05). After treatment, the serum levels of visceral fat-specific serine protease inhibitor (Vaspin) and nitric oxide (NO) in the combination group were higher than those in the monotherapy group, while the serum level of retinol-binding protein 4 (RBP4) was lower, with statistically significant differences (P < 0.05). After treatment, the sensory nerve conduction velocity (SNCV) and motor nerve conduction velocity (MNCV) of the median nerve and common peroneal nerve in the combination group were higher than those in the monotherapy group, with statistically significant differences (P < 0.05). After treatment, the TCM syndrome score in the combination group was lower than that in the monotherapy group, with statistically significant differences (P < 0.05). The total effective rate of clinical treatment in the combination group was higher than that in the monotherapy group, with statistically significant differences (P < 0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion: Modified Huangqi Guizhi Wuwu Decoction combined with empagliflozin shows significant efficacy and good safety in the treatment of T2DM, and can significantly alleviate oxidative stress, improve vascular endothelial function, enhance peripheral nerve conduction velocity, and relieve TCM syndrome symptoms.
