Evaluation of Collateral Circulation in Elderly Patients with Cerebral Infarction Using Multimodal Computed Tomography Imaging Technology
Abstract:Objective: To explore the value of multimodal computed tomography (CT) imaging technology in evaluating collateral circulation in elderly patients with cerebral infarction. Methods: Clinical data of 96 elderly patients with cerebral infarction admitted to Xinyang Central Hospital from March 2022 to March 2024 were collected. All patients underwent digital subtraction angiography (DSA) and multimodal CT examination after admission. Based on the DSA results, patients were divided into two groups: the group with good collateral circulation and the group with poor collateral circulation. The multimodal CT parameters of the two groups were compared, and receiver operating characteristic (ROC) curves were drawn to analyze the value of multimodal CT parameters in assessing collateral circulation in patients with cerebral infarction. Results: Among the 96 elderly patients with cerebral infarction, 51 were classified into the group with good collateral circulation, and 45 into the group with poor collateral circulation. The cerebral blood flow (CBF), cerebral blood volume (CBV), and regional leptomeningeal collateral circulation (rLMC) scores were lower in the poor collateral circulation group compared to the good collateral circulation group, while the mean transit time (MTT) and time to peak (TTP) were longer in the poor collateral circulation group, with statistically significant differences (P < 0.05). ROC curves showed that the areas under the curve (AUC) for individual and combined assessments of collateral circulation in cerebral infarction using CBF, CBV, MTT, TTP, and rLMC scores were 0.727, 0.708, 0.744, 0.754, 0.838, and 0.948 (P < 0.001). Conclusion: Multimodal CT imaging technology has certain diagnostic value in evaluating the collateral circulation status of elderly patients with cerebral infarction, and the combined assessment of multimodal CT parameters has higher value.
