Predictive Value of Alpha-Fetoprotein and Carbohydrate Antigen 724 for Lymph Node Metastasis in Patients with Early Gastric Cancer
Abstract：Objective: To explore the predictive value of alpha-fetoprotein (AFP) and carbohydrate antigen 724 (CA724) for lymph node metastasis in patients with early gastric cancer. Methods: Data from 84 patients with early gastric cancer admitted to the Liangyuan District TCM Hospital in Shangqiu City from December 2020 to December 2024 were included. Patients were divided into a metastasis group (21 cases) and a non-metastasis group (63 cases) based on their lymph node metastasis status. Clinical data and levels of AFP and CA724 at admission were collected and analyzed to assess the predictive value of AFP and CA724 for lymph node metastasis. Results: The proportion of poorly differentiated tumors, serum tumor necrosis factor-α (TNF-α), AFP, and CA724 levels at admission were higher in the metastasis group compared to the non-metastasis group, with statistically significant differences (P < 0.05). The degree of differentiation, serum TNF-α, AFP, and CA724 levels at admission had certain predictive value for the risk of lymph node metastasis in patients with early gastric cancer. The area under the receiver operating characteristic (ROC) curve (AUC) was greater than 0.70. A nomogram prediction model was constructed based on the degree of differentiation at admission, TNF-α, and the values of AFP and CA724. This model showed an AUC of 0.971 in predicting the risk of lymph node metastasis in patients with early gastric cancer. Conclusion: AFP and CA724 have some predictive value for the risk of lymph node metastasis in patients with early gastric cancer. The nomogram prediction model combined with other clinical indicators can visualize the risk and improve the predictive performance.
