Evaluation of the Diagnostic Efficiency of Glycated Hemoglobin Combined with Fasting Blood Glucose and Glycated Albumin in Community Diabetes Screening
Abstract:Objective: To investigate the diagnostic value of combined detection of glycated hemoglobin (HbA1C), fasting blood glucose (FBG), and glycated albumin (GA) for diabetes, and to evaluate their application efficiency in community medical institutions. Methods: A total of 788 diabetic suspects who visited the Jinshan Street Community Health Service Center in Huli District, Xiamen City from September 2024 to April 2025 were selected as the study subjects. All participants underwent HbA1C, FBG, GA testing, and oral glucose tolerance test (OGTT). Taking the OGTT results as the reference standard, the sensitivity, specificity, accuracy, and area under the receiver operating characteristic curve (AUC) of each single indicator and different combination schemes were calculated, and their diagnostic efficiency was compared. Results: Among the 788 diabetic suspects, 237 cases (30.08%) were positive, and 551 cases (69.92%) were negative. The sensitivity and accuracy of the combined detection of HbA1C, FBG, and GA were higher than those of single or two-by-two combinations. The AUC of the three combined detection methods was higher than that of single or two-by-two combinations, with statistically significant differences (P < 0.05). Conclusion: In community diabetes screening, the combined detection of HbA1C, FBG, and GA can significantly improve diagnostic efficiency and has certain complementary value, making it suitable for scenarios where OGTT is restricted.
