Value of Coronary Computed Tomography Angiography in the Diagnosis of Coronary Heart Disease and Prediction of Major Adverse Cardiovascular Events
Abstract:Objective: To investigate the value of coronary computed tomography angiography (CCTA) in the diagnosis of coronary heart disease and the prediction of major adverse cardiovascular events (MACE). Methods: A retrospective analysis was conducted on 150 patients suspected of having coronary heart disease admitted to Zhumadian Guangji Cardiovascular Hospital from October 2022 to January 2024. Electronic medical records and hospital information system data were reviewed to collect relevant patient information. Coronary angiography was used as the 'gold standard' to evaluate the diagnostic efficacy of CCTA in patients with coronary heart disease. Meanwhile, a 6-month follow-up was conducted on the study subjects to record the occurrence of MACE, thereby exploring the predictive value of CCTA in MACE. Results: Among the 150 study subjects, 94 were finally diagnosed with coronary heart disease through coronary angiography. The sensitivity, specificity, and accuracy of CCTA in diagnosing coronary heart disease were 97.87%, 100.00%, and 98.67%, respectively. Among the 94 diagnosed patients, 39 eventually experienced MACE, while 55 did not. Comparison of CCTA-related parameters between the two groups showed that the MACE group had significantly higher coronary artery stenosis degree, total plaque volume, and plaque burden compared to the non-MACE group. Additionally, the coronary CT fractional flow reserve (CT-FFR) was significantly lower in the MACE group, with statistically significant differences (P < 0.05). Logistic regression analysis revealed that indicators such as coronary artery stenosis degree, CT-FFR, total plaque volume, and plaque burden were significantly associated with the occurrence of MACE in patients with coronary heart disease (P < 0.05). Conclusion: CCTA has high sensitivity, specificity, and accuracy in the detection of coronary heart disease. Some CCTA parameters are closely related to whether patients with coronary heart disease experience MACE, and it is worth clinical application.
