Diagnostic Value of Antiphospholipid Antibodies, Proteinase 3 Antibodies, Anti-Thyroid Peroxidase Antibodies, Mean Platelet Volume, and Platelet Distribution Width in Pediatric Autoimmune Hemolytic Anemia
Abstract:Objective: To explore the diagnostic value of antiphospholipid antibodies (aPLs), proteinase 3 (PR3) antibodies, anti-thyroid peroxidase (TPO) antibodies, mean platelet volume (MPV), and platelet distribution width (PDW) in pediatric autoimmune hemolytic anemia (AIHA). Methods: A total of 80 AIHA children admitted to Nanyang Central Hospital from May 2022 to April 2025 were selected as the study subjects and divided into primary (40 cases) and secondary groups (40 cases) according to the etiology. The positivity of aPLs, PR3 antibodies, serum levels of anti-TPO antibodies, and PDW, MPV were detected and compared between the two groups. Spearman correlation analysis was used to evaluate the relationship between these indicators and AIHA subtypes. Receiver operating characteristic (ROC) curves were drawn to assess the diagnostic efficacy of each indicator individually and in combination. Results: The positivity rates of aPLs and PR3 antibodies, as well as MPV and PDW, were higher in the secondary group than in the primary group, while the serum anti-TPO antibody levels were lower in the secondary group, with statistically significant differences (P < 0.05). Spearman correlation analysis showed that aPLs positivity, PR3 antibodies, MPV, and PDW were positively correlated with secondary AIHA (r > 0, P < 0.05), whereas anti-TPO antibodies were negatively correlated with secondary AIHA (r < 0, P < 0.05). ROC curve analysis showed that the area under the curve (AUC) of aPLs, PR3 antibodies, anti-TPO antibodies, MPV, and PDW were all greater than 0.750. When combined, the AUC increased to 0.917, with sensitivity and specificity of 0.950 and 0.750, respectively. Conclusion: Secondary AIHA patients showed positive aPLs and PR3 antibodies. Combined detection of anti-TPO antibodies and platelet parameters MPV and PDW can significantly improve the diagnostic efficiency.
