Clinical Study on the Treatment of Ovarian Dysfunction with Follicular Development Disorders in Patients with Kidney Yin Deficiency Type by Modified Zuo Gui Wan Combined with Dietary Therapy
Abstract:Objective: To investigate the clinical efficacy of modified Zuo Gui Wan combined with follicle-nourishing soup for the treatment of infertility due to follicular development disorders in patients with kidney yin deficiency type ovarian insufficiency (DOR). Methods: A total of 56 patients with DOR and follicular development disorders were selected from the Department of Traditional Chinese Gynecology (including the Famous Physicians' Clinic) of Fuyong Maternal and Child Health Hospital, Shenzhen City, from January 2023 to December 2024. They were randomly divided into a treatment group (modified Zuo Gui Wan + follicle-nourishing soup + letrozole) and a control group (placebo TCM dietary therapy + letrozole), with 28 cases in each group. The changes in TCM syndrome scores, baseline ovarian hormone indicators [basic follicle-stimulating hormone (bFSH), basic luteinizing hormone (bLH), anti-Müllerian hormone (AMH), estradiol (E2)], follicular conditions, pregnancy outcomes, and overall clinical efficacy were compared between the two groups before and after treatment. Results: After treatment, the TCM syndrome scores and total scores of the treatment group were significantly lower than those before treatment and those of the control group (P < 0.05). After treatment, the serum levels of bFSH and bLH in the treatment group were significantly lower than those before treatment and those of the control group, while the serum levels of E2 and AMH were significantly higher than those before treatment and those of the control group (P < 0.05). After treatment, the total number of antral follicles (AFC) and the number of mature follicles in the treatment group were significantly higher than those before treatment and those of the control group, while the difference in follicle length and diameter was smaller than that before treatment and that of the control group (P < 0.05). The pregnancy rate and overall effective rate of the treatment group were higher than those of the control group, with statistically significant differences (P < 0.05). Conclusion: Modified Zuo Gui Wan combined with follicle-nourishing soup can effectively improve follicular development in patients with DOR-related infertility due to follicular development disorders, increase the pregnancy rate, and alleviate symptoms such as menstrual irregularities and lumbar and knee soreness.
