Application Effect of Sintilimab Combined with Paclitaxel and Carboplatin Neoadjuvant Chemotherapy in Patients with Stage III Squamous Non-Small Cell Lung Cancer
Abstract:Objective: To investigate the application effect of sintilimab combined with paclitaxel and carboplatin neoadjuvant chemotherapy in patients with stage III squamous non-small cell lung cancer (NSCLC). Methods: A total of 110 patients with stage III squamous NSCLC admitted to the First People's Hospital of Nanyang City from March 2022 to June 2024 were selected as the study subjects. According to the random number table method, they were divided into the neoadjuvant group (paclitaxel and carboplatin neoadjuvant chemotherapy) and the monoclonal antibody combination group (sintilimab combined with paclitaxel and carboplatin neoadjuvant chemotherapy), with 55 cases in each group. The clinical efficacy, serum tumor marker levels, inflammatory-related indicators, immune function, and incidence of adverse reactions before and after treatment were compared between the two groups. Results: The overall disease control rate in the monoclonal antibody combination group was 89.09%, which was higher than 74.55% in the neoadjuvant group, with a statistically significant difference (P < 0.05). After treatment, the levels of serum carbohydrate antigen 125 (CA-125), carcinoembryonic antigen (CEA), neuron-specific enolase (NSE), and cytokeratin 19 fragment antigen (CYFRA21-1) in the monoclonal antibody combination group were lower than those in the neoadjuvant group, with statistically significant differences (P < 0.05). After treatment, the peripheral blood cluster of differentiation (CD) 4+ level and CD4+/CD8+ ratio in the monoclonal antibody combination group were higher than those in the neoadjuvant group, while the CD8+ level was lower, with statistically significant differences (P < 0.05). After treatment, the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammatory index (SII) in the monoclonal antibody combination group were lower than those in the neoadjuvant group, with statistically significant differences (P < 0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P > 0.05). Conclusion: Sintilimab combined with paclitaxel and carboplatin neoadjuvant chemotherapy has a better effect, which can reduce the serum tumor marker levels, inhibit inflammatory responses, improve immune function, and does not increase adverse reactions.
