Effect of Different Types of Exhalation Valves on Carbon Dioxide Clearance and Oxygen Partial Pressure Improvement in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease (AECOPD) Combined with Type II Respiratory Failure
Abstract:Objective: To investigate the effect of different types of exhalation valves on carbon dioxide clearance and oxygen partial pressure improvement in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) combined with type II respiratory failure. Methods: A total of 90 patients with AECOPD combined with type II respiratory failure who were hospitalized at Haizhu District TCM Hospital in Guangzhou from January 2023 to December 2024 were selected as the study subjects. All patients received non-invasive ventilator treatment and were divided into three groups based on the type of exhalation valve used: side-hole valve group, platform valve group, and mask-integrated exhalation valve group, with 30 patients in each group. According to the degree of pre-treatment carbon dioxide elevation, the patients were further divided into mild-to-moderate elevation subgroup and severe elevation subgroup. The oxygen partial pressure (PO2) and carbon dioxide partial pressure (PCO2) before and after treatment were compared among the three groups. Results: After treatment, PO2 was higher than before treatment at 2 h, 24 h, and 48 h, with statistically significant differences (P < 0.05). For the mild-to-moderate elevation subgroup, PCO2 was lower than before treatment at 2 h, 24 h, and 48 h, and decreased further over time, with statistically significant differences (P < 0.05). For the severe elevation subgroup, PCO2 was also lower than before treatment at 2 h, 24 h, and 48 h, and decreased further over time, with statistically significant differences (P < 0.05). At 2 h, 24 h, and 48 h after treatment, the PCO2 of the severe elevation subgroup in the platform valve group was lower than that in the side-hole valve group and the mask-integrated exhalation valve group, with statistically significant differences (P < 0.05). Conclusion: For patients with mild-to-moderate elevation of PCO2, the difference in carbon dioxide clearance effect among different exhalation valves is small. However, for patients with severe elevation of PCO2, the platform-type exhalation valve can clear carbon dioxide more rapidly and effectively than other types of exhalation valves.
