Meta-analysis of Randomized Controlled Trials Comparing Pleurectomy and Pleural Friction for the Treatment of Pneumothorax
Abstract:Objective To systematically evaluate the efficacy of pleurectomy versus pleural friction in the treatment of pneumothorax. Methods A computerized search was conducted in the CNKI database, PubMed database, Cochrane Library, and EMBASE database to collect published randomized controlled trials (RCTs) on the treatment of pneumothorax with pleurectomy and pleural friction, with a retrieval period from the establishment of the databases to January 1, 2025. After screening the literature, extracting data, and assessing the risk of bias in the included studies, a Meta-analysis was performed using RevMan 5.4.1 software. Results A total of 10 RCTs were included, involving 1,137 patients with pneumothorax (662 in the pleurectomy group and 475 in the pleural friction group). The Meta-analysis results showed that compared with pleural friction, pleurectomy had an advantage in postoperative ipsilateral pneumothorax recurrence [OR=0.31, 95%CI(0.14,0.69), P=0.004]; however, there were no statistically significant differences between the two groups in postoperative hospital stay [SMD=-0.15, 95%CI(-0.66,0.37), P=0.57], operation time [SMD=0.17, 95%CI(-1.40,1.73), P=0.83], and postoperative tube removal time [SMD=0.10, 95%CI(-0.37,0.58), P=0.67]. Conclusion Current evidence suggests that pleurectomy can better improve the treatment effect of pneumothorax and reduce the recurrence rate. Clinically, for patients with recurrent or refractory pneumothorax, pleurectomy is recommended as the first choice.
