Study on the Preventive Effect of Stratified Nursing Based on Hepatic Venous Pressure Gradient Monitoring on Rebleeding in Patients with Esophageal and Gastric Varices due to Cirrhosis
Abstract:Objective: To explore the preventive effect of stratified nursing based on hepatic venous pressure gradient (HVPG) monitoring on rebleeding in patients with esophageal and gastric varices due to cirrhosis, combined with dynamic risk adjustment and resource allocation plans. Methods: A total of 106 patients with esophageal and gastric varices due to cirrhosis were randomly divided into an observation group and a control group, each with 53 cases. The observation group received stratified nursing according to HVPG monitoring results, i.e., patients were stratified based on HVPG values (high-risk group with ≥20 mmHg, medium-low-risk group with <20 mmHg), and nursing plans were dynamically adjusted. The control group received routine nursing. The rebleeding rate, complication rate, and nursing satisfaction between the two groups were compared to evaluate the effectiveness of different nursing models. Results: The rebleeding rate in the observation group was 9.43%, significantly lower than 28.30% in the control group (P<0.05). In the high-risk group of the observation group, the incidence of thrombosis was 4.55%, significantly lower than 20.75% in the control group (P<0.05). The nursing satisfaction score of the observation group [(92.40±5.10) points] was significantly higher than that of the control group [(80.30±7.60) points, P<0.01]. Conclusion: Stratified nursing based on HVPG monitoring combined with dynamic risk adjustment and resource allocation plans can effectively reduce the risk of rebleeding in patients with esophageal and gastric varices due to cirrhosis, significantly improving nursing quality and patient satisfaction.
