Potential Profile Classification and Influencing Factors Analysis of Dyspnea Fear in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease (COPD)
Abstract:Objective: To analyze the potential profile classification of dyspnea fear in patients with acute exacerbation of chronic obstructive pulmonary disease (COPD) and its influencing factors. Methods: A prospective study was conducted from May 2021 to May 2024, selecting 240 hospitalized patients with acute exacerbation of COPD as the research subjects. The Breathlessness Beliefs Questionnaire (BBQ) was used to assess the level of dyspnea fear in COPD patients. Latent profile analysis was applied to determine the optimal number of dyspnea fear categories and name them. Clinical data were collected through the hospital electronic medical record system. Univariate analysis and multivariate logistic regression analysis were performed to explore the influencing factors of different dyspnea fear categories. Results: Latent profile analysis showed that the dyspnea fear of 240 hospitalized patients with acute exacerbation of COPD could be classified into three latent categories: low dyspnea fear (61 cases, 25.4%), moderate dyspnea fear (113 cases, 47.1%), and high dyspnea fear (66 cases, 27.5%). Univariate analysis and multivariate logistic regression analysis results showed that body mass index (BMI), smoking cessation status, modified British Medical Research Council dyspnea scale (mMRC) grade, breathlessness catastrophic cognition scale (BCS) score, COPD self-efficacy scale (CSES) score, Hamilton Depression Scale (HAMD) score, and Hamilton Anxiety Scale (HAMA) score were independent influencing factors for different dyspnea fear categories (P < 0.05). Conclusion: There is a certain heterogeneity in the potential profile classification of dyspnea fear in patients with acute exacerbation of COPD, which is influenced by factors such as BMI, smoking cessation status, mMRC grade, and BCS score. This suggests that medical staff can classify patients' dyspnea fear levels in detail based on these influencing factors and develop personalized nursing measures to reduce patients' dyspnea fear levels.
