Application of Staged Focused Nursing in Patients with Decompensated Hepatitis B-Related Cirrhosis
Abstract:Objective: To explore the effects of staged focused nursing on liver function, nutritional status, psychological state, and self-care ability of patients with decompensated hepatitis B virus-related cirrhosis (HBV-DC). Methods: A total of 114 HBV-DC patients admitted to the hospital from November 2021 to October 2023 were selected as the study subjects and randomly divided into a conventional nursing group and a focused nursing group, with 57 cases in each group. The conventional nursing group received routine nursing intervention, while the focused nursing group received staged focused nursing on top of conventional nursing. The levels of liver function indicators [total bilirubin (TBIL), aspartate aminotransferase (AST), alanine aminotransferase (ALT)], improvement in nutrition [serum prealbumin (PA), albumin (ALB)], incidence of complications, disease knowledge level, psychological resilience [Connor-Davidson Resilience Scale (CD-RISC)], fear of disease progression [Fear of Progression Questionnaire (FoP-Q)], self-care ability [Adult Health Self-Management Scale (AHSMSRS)], and quality of life [Quality of Life Inventory (GQOLI-74)] were compared between the two groups after intervention. Results: After intervention, the levels of serum TBIL, AST, and ALT in the focused nursing group were lower than those in the conventional nursing group, while the levels of serum PA and ALB were higher (P < 0.05). The total complication rate in the focused nursing group (3.51%) was lower than that in the conventional nursing group (15.79%), and the excellent and good disease knowledge rate in the focused nursing group (98.25%) was higher than that in the conventional nursing group (82.46%) (P < 0.05). After intervention, the CD-RISC, AHSMSRS, and GQOLI-74 scores of the focused nursing group were higher than those of the conventional nursing group, while the FoP-Q score was lower (P < 0.05). Conclusion: Implementing staged focused nursing intervention can improve the disease knowledge level and psychological resilience of HBV-DC patients, reduce fear of disease progression, enhance self-management ability, reduce the incidence of complications, and thus improve liver function and nutritional status, as well as quality of life.
