Construction and Application of an Evaluation Index System for Airway Nursing Quality After Laryngeal Cancer Tracheostomy
Abstract:Objective: To construct an evaluation index system for airway nursing quality after laryngeal cancer tracheostomy and to test its application effect. Methods: Through literature analysis and the three-dimensional quality model of structure-process-outcome, airway nursing quality evaluation indicators after laryngeal cancer tracheostomy were collected and organized. The indicators were screened and improved through expert questionnaires, and the corresponding index system was established. Since January 2022, the evaluation index system for airway nursing quality after laryngeal cancer tracheostomy was applied. Two stages of patients with laryngeal cancer who underwent tracheostomy were selected as research subjects: the period from January to December 2021 and the period from January to December 2022. They were divided into the control group and the observation group according to the application of the index system, with 54 cases in each group. The airway management effect and nursing satisfaction were compared between the two groups. Results: The recovery rate of the two rounds of questionnaires was 100% each. The expert authority coefficients (Cr) were 0.856 and 0.887, respectively. Kendall's coefficient of concordance were 0.427 (x2=35.297, P<0.001) and 0.491 (x2=50.283, P<0.001), with statistically significant differences (P<0.05). The evaluation index system for airway nursing quality after laryngeal cancer tracheostomy included 3 first-level indicators, 12 second-level indicators, and 43 third-level indicators, with a coefficient of variation ranging from 0.084 to 0.157. The application results showed that the incidence of complications in the observation group was lower than that in the control group, and the airway humidification effect in the observation group was better than that in the control group (P<0.05). The nursing satisfaction evaluation in the observation group was higher than that in the control group (P<0.05). Conclusion: The indicators involved in the evaluation index system for airway nursing quality after laryngeal cancer tracheostomy cover multiple aspects of postoperative airway nursing management, which is relatively comprehensive and has certain scientific and reliable characteristics. Implementing nursing management based on this index system helps to standardize clinical nursing, continuously improve nursing quality, and thus enhance airway management effectiveness and nursing satisfaction.
