Application Effect of Nursing Intervention Based on the Chronic Illness Trajectory Framework in Patients with Diabetic Nephropathy
Abstract:Objective: To explore the application effect of nursing intervention based on the Chronic Illness Trajectory Framework (CITF) in patients with diabetic nephropathy (DN). Methods: From August 2022 to August 2023, 130 patients with DN were selected as the study subjects. According to the random number table method, the patients were divided into the observation group and the control group, with 65 cases in each group. The control group received routine nursing care, while the observation group received nursing intervention based on CITF. Before and after the intervention, the patients' self-care ability and quality of life were evaluated using the Empowerment Scale for Chronic Illness (ESCA) and the Short Form Health Survey (SF-36), and the dialysis compliance and incidence of complications between the two groups were compared. Results: After the intervention, the self-care ability, dialysis compliance, and quality of life scores of the observation group were higher than those of the control group (P < 0.05). The incidence of dialysis-related complications in the observation group was lower than that in the control group (P < 0.05). Conclusion: Nursing intervention based on CITF can enhance the self-care ability and dialysis compliance of patients with DN, effectively prevent dialysis-related complications, and improve the quality of life of patients.
