Analysis of Factors Affecting Ureteral Stent-Related Symptoms in Patients with Upper Urinary Tract Stones After Placement of a Double-J Tube and Nursing Interventions
Abstract:Objective: To analyze the factors affecting ureteral stent-related symptoms in patients with upper urinary tract stones after placement of a double-J tube. Methods: A retrospective analysis was conducted on 175 patients with upper urinary tract stones who had a double-J tube placed post-surgery between June 2020 and June 2023. Patients were divided into two groups based on whether they experienced ureteral stent-related symptoms before and after surgery. Clinical data were collected, and univariate analysis and binary logistic regression were used to identify factors influencing the occurrence of ureteral stent-related symptoms. A risk prediction model was established, and its predictive performance was evaluated using ROC curves. Results: In this study, 76 patients were in the occurrence group, and 99 patients were in the non-occurrence group. Multivariate analysis showed that older age [OR=1.048, 95%CI(1.005,1.092), P=0.028], hypertension [OR=3.373, 95%CI(1.432,7.946), P=0.005], diabetes [OR=4.461, 95%CI(1.418,14.030), P=0.011], and postoperative daily water intake <1500 mL [OR=2.300, 95%CI(1.106,4.782), P=0.026] were independent risk factors for ureteral stent-related symptoms in patients with upper urinary tract stones after surgery (P<0.05). A risk prediction model was established: Logistic(P)=0.046×age + 1.216×hypertension + 1.495×diabetes + 0.833×postoperative daily water intake - 4.212. The discriminative ability of the early warning model was analyzed using the receiver operating characteristic (ROC) curve. The area under the ROC curve (AUC) of the constructed model was 0.775 [95%CI(0.706, 0.843)], and the Hosmer-Lemeshow test showed χ²=3.491, P=0.900. Conclusion: Clinically, based on the above independent risk factors and referring to the risk prediction model established in this study, personalized nursing strategies can be developed to reduce the occurrence of ureteral stent-related symptoms.
