Review Indicators for Preventing Recurrence of Diabetic Foot and Analysis of Obstacle Factors

Abstract：Objective: To understand the gap between the best evidence for preventing recurrence of diabetic foot and clinical practice, analyze the obstacle factors in applying the evidence, implement practice changes, and provide references for clinical practice. Methods: First, determine the evidence-based problem, then conduct literature search, literature inclusion, evidence extraction, and finally develop review indicators and methods. Based on the baseline review results, analyze the obstacle factors and develop improvement measures. Results: A total of 14 pieces of best evidence were included, and 13 review indicators were developed. Among them, 12 review indicators had an adherence rate of less than 60%. The obstacle factors include the difficulty in establishing a multidisciplinary team for preventing recurrence of diabetic foot due to the large number of involved personnel, nurses' low level of knowledge and awareness regarding the prevention of diabetic foot recurrence, lack of assessment scales for preventing diabetic foot recurrence in the department, and incomplete educational materials for diabetic foot prevention within the department. Practice change strategies include strengthening leadership support, conducting training on the prevention of diabetic foot recurrence, embedding the risk factor classification system (IWGDF 2015 edition) into the nursing record system, and improving the educational materials for preventing diabetic foot recurrence. Conclusion: There is a significant difference between the review indicators for preventing diabetic foot recurrence and clinical practice. Clinical staff should analyze the obstacle factors based on the actual situation of their departments and propose reasonable intervention measures to promote evidence translation.
