Current Status and Influencing Factors of Fluid Intake Adherence in Hemodialysis Patients: A Meta-Analysis
Abstract:Objective: To analyze the current status of fluid intake adherence in hemodialysis patients and its influencing factors, providing a reference for improving patients' fluid intake adherence. Methods: Computerized retrieval was conducted from the Cochrane Library, Web of Science, Embase, PubMed, SinoMed, CNKI, Wanfang Database, and VIP Database to search cross-sectional studies on the influencing factors of fluid intake adherence in hemodialysis patients, with the retrieval time from the establishment of the database to July 31, 2023. Two researchers independently selected the literature according to inclusion and exclusion criteria, performed quality evaluation and data extraction. Meta-analysis was conducted using STATA 12.0 software and RevMan 5.4 software. Results: A total of 13 studies were included, with a sample size of 1,910 cases. Among them, 914 cases had good fluid intake adherence. The fluid intake adherence rate of hemodialysis patients was 48% [95% CI (34%, 62%)]. The literature quality evaluation scores were all ≥4 points, indicating medium or higher quality. The meta-analysis showed that the influencing factors of fluid intake adherence in hemodialysis patients included age [OR=2.44, 95% CI (1.80, 3.31), P<0.01; OR=2.36, 95% CI (1.42, 3.94), P<0.01], residual urine volume [OR=1.85, 95% CI (1.11, 3.05), P=0.02], intention to adhere to fluid intake [OR=5.11, 95% CI (3.10, 8.43), P<0.01], presence of caregivers [OR=8.33, 95% CI (2.41, 28.75), P<0.01], and type of caregiver [OR=3.19, 95% CI (1.27, 8.01), P=0.01; OR=8.39, 95% CI (3.37, 20.86), P<0.01]. Conclusion: Age, intention to adhere to fluid intake, residual urine volume, presence of caregivers, and type of caregiver influence the fluid intake adherence of hemodialysis patients. Therefore, researchers should identify the influencing factors of fluid intake adherence as early as possible and take targeted measures to improve patient adherence.
