Effect of Zishen Yutai Pills Combined with Dydrogesterone on Clinical Symptoms and Progesterone Levels in Patients with Early Threatened Abortion
Abstract:Objective: To explore the effect of Zishen Yutai Pills combined with dydrogesterone on clinical symptoms and progesterone levels in patients with early threatened abortion. Methods: A total of 85 patients with early threatened abortion admitted to the Nankang District Hospital of Traditional Chinese Medicine in Ganzhou City from January 2022 to January 2023 were selected as the study subjects and randomly divided into an experimental group (n=42) and a conventional group (n=43). The conventional group was treated with dydrogesterone, while the experimental group received Zishen Yutai Pills in addition to the conventional treatment. After treatment until week 12 of pregnancy, the improvement of clinical symptoms, progesterone and serum human chorionic gonadotropin (β-HCG) levels, Th1/Th2-related cytokine levels [tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10), gamma-interferon (IFN-γ)], coagulation function [fibrinogen (FIB), prothrombin time (PT), activated partial thromboplastin time (APTT), D-dimer levels], uterine artery blood flow parameters [resistance index (RI), peak systolic velocity (PSV), and pulsatility index (PI)], pregnancy outcomes, and adverse reactions were compared between the two groups. Results: The time for alleviation of abdominal pain, lumbar soreness, and vaginal bleeding was shorter in the experimental group than in the conventional group, with statistically significant differences (t=7.074, 9.314, 5.402, P<0.05). After treatment, both groups showed increased progesterone and serum β-HCG levels, and the experimental group had higher levels of progesterone and β-HCG than the conventional group, with statistically significant differences (t=5.218, 11.729, P<0.05). After treatment, the levels of TNF-α and IFN-γ decreased, while IL-10 levels increased in both groups, and the experimental group had higher IL-10 levels and lower TNF-α and IFN-γ levels than the conventional group, with statistically significant differences (t=6.722, 4.959, 7.137, P<0.05). After treatment, PT and APTT were prolonged, FIB and D-dimer levels were reduced in both groups, and the experimental group had longer PT and APTT, and lower FIB and D-dimer levels than the conventional group, with statistically significant differences (t=3.537, 3.466, 3.063, 2.887, P<0.05). After treatment, RI and PI decreased, while PSV increased in both groups, and the experimental group had lower RI and PI, and higher PSV than the conventional group, with statistically significant differences (t=9.545, 2.994, 5.826, P<0.05). The abortion rate in the experimental group was lower, and the term pregnancy rate was higher than that in the conventional group, with statistically significant differences (χ²=4.930, 3.943, P<0.05). There was no statistically significant difference in the preterm birth rate between the two groups (χ²=0.001, P>0.05). The Apgar score of newborns in the experimental group was higher than that in the conventional group, with statistically significant differences (t=13.923, P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (χ²=0.113, P=0.736). Conclusion: Zishen Yutai Pills combined with dydrogesterone can improve progesterone and β-HCG levels, correct the imbalance of Th1/Th2 cytokines, improve coagulation function and uterine artery blood supply, accelerate the improvement of clinical symptoms, and enhance pregnancy outcomes, with high safety.
